This is to be kept on each dog that is AKC registered that you breed or
sell per AKC field inspectors
These forms are to be kept on each female dog that produces a litter of
puppies akc now furnishes these forms already with sire and dams name
printed on them but you can also use this form if you are breeding any female
dog to keep in contact with new owners
Also just so you know Field inspector's or AKC require you to permanently identify your animals if you have
two of the same sex and breed at the same location. So if you have two female Rotties they need to be
tattooed or microchiped according to field inspectors-tags will not due they say collars can come off, Also I
always recommend keeping more than the required documents like if you have to revolk limited
registration I keep a copy of those papers, if you ever have to write AKC for lost paperwork I keep copies of
that letter sent to them, If they ever send me a letter I keep a copy of that. Each dog has their own 3 ring
binder first page is always the form at the top of this page, then all registration papers akc ckc ukc etc, then
pedigree, then health certs and dna, then pics of dogs in the pedigreee and at the back of it all the litters
they produced on this form above. All the pages are in clear plastic covers to protect the wording on the
forms. This makes it easy for a field inspection they ask about a dog or a litter you can pull that dogs folder
and it will be like Ragu IN THERE!!!!!!
